Please complete the following forms and review the required documents before participating in your tour.
Please submit the Participant Information Form to confirm your reservation. The Diver Medical Form must be completed and submitted no later than two weeks before your departure.

RequiredAll guests participating in diving activities with DayDream Palau are required to complete a Diver Medical Form.
Please complete and submit the form no later than two weeks before your departure.
If you answer “Yes” to Question 3, 5 or 10 on page 1, or to any question on page 2, you must obtain a medical evaluation and approval from a physician.
Please refer to the instructions included in the medical form for details.

Guests boarding Ryoma I must review this document before boarding the vessel.
